Spots are limited and we often have a waitlist. If you need to cancel or will miss your session, please let us know as soon as possible so we can offer your spot to another player.

We reserve the right to cancel a session if the minimum number of participants is not met. In that case, a full refund will be issued.

🏐 Volleyball Clinic Waiver, Release of Liability, and Media Consent

Please read carefully before signing. This is a legal document.

1. 🛡 Participation Waiver and Release of Liability

I, the undersigned parent or legal guardian of the above-named minor ("Athlete"), give permission for the Athlete to participate in a volleyball clinic hosted by Coach Chijo LLC(the “Clinic”).

I understand that athletic activities involve inherent risks, including but not limited to physical injury, accidents, or illness. I voluntarily accept full responsibility for any risks, injuries, damages, or losses that may occur as a result of the Athlete’s participation.

To the fullest extent allowed by law, I hereby release and hold harmless Coach Chijo LLC, and their employees, contractors, coaches, volunteers, agents, and affiliates from any and all claims or liabilities—whether caused by negligence or otherwise—arising from the Athlete’s participation in the Clinic.

2. 📸 Media Release and Use of Likeness

I understand that Coach Chijo LLC may photograph, film, or otherwise record the Athlete during Clinic activities. I grant Coach Chijo LLC —and their legal representatives, licensees, assigns, and affiliates—the unrestricted, royalty-free, and perpetual right to use any media they individually capture of the Athlete’s image, voice, and likeness in any format or media, including but not limited to:

  • Social media (e.g., Instagram, TikTok, Facebook, YouTube)
  • Promotional or educational materials
  • Websites, advertising, and newsletters
  • Online courses, webinars, or digital products

Coach Chijo LLC retains full ownership and usage rights to the content they produce. This media may be used for commercial, marketing, instructional, or other lawful purposes. I understand that no compensation will be provided for the use of such content.

3. ✅ Consent and Acknowledgment

By signing this document, I confirm that:

  • I am the legal parent or guardian of the above-named Athlete.
  • I have read and understand the Waiver and Media Release above.
  • This agreement is binding on me, the Athlete, and our heirs or legal representatives.
  • I have full legal authority to provide this consent.
