Lesson Rate: $150 (55-minute lesson)

1. Participation Waiver and Release of Liability
I, the undersigned parent/legal guardian of the above-named minor ("Athlete"), hereby give permission for the Athlete to participate in the volleyball camp hosted by Coach Chijo LLC ("Camp"). I understand that participation in any athletic activity involves the risk of injury. I assume full responsibility for any and all injuries, losses, or damages that may occur to the Athlete as a result of participation.

I hereby release, waive, discharge, and hold harmless Coach Chijo LLC and Ikaika Ohana Volleyball its staff, coaches, volunteers, affiliates, and agents from any and all claims for injuries, damages, liabilities, or losses arising out of or related to participation in this Camp.

2. Media Release and Commercial Use
I understand and agree that Coach Chijo LLC may photograph, video record, or otherwise capture the likeness, image, voice, and performance of the Athlete during the Camp.

I grant Coach Chijo LLC, and its legal representatives, successors, assigns, licensees, and designees the irrevocable, royalty-free, and perpetual right and license to use, reproduce, edit, display, publish, and distribute these recordings or images, in whole or in part, for any lawful commercial purpose, including but not limited to:

Social media platforms (e.g., Instagram, TikTok, Facebook, YouTube)
Online advertising
Websites and marketing materials
Webinars, online courses, and other instructional or promotional media
Any future commercial or educational products produced by Coach Chijo LLC

I acknowledge that I will receive no financial compensation for the use of such media, and that all rights, title, and interest in such materials belong exclusively to Coach Chijo LLC.

3. Consent and Acknowledgment
By signing below, both the Athlete and Parent/Guardian acknowledge and agree to all the terms stated above. The Parent/Guardian affirms that they have the legal authority to provide this consent on behalf of the minor child.
