🏐 Volleyball Clinic Audit Waiver, Release of Liability, and Media Consent

Please read carefully before signing. This is a legal document.

1. 🛡 Participation Waiver and Release of Liability

I, the undersigned coach ("Auditing Coach"), request permission to observe and audit a volleyball clinic hosted by Coach Chijo LLC (the "Clinic") for the purpose of my own professional development.

I understand that I am attending solely as an independent observer, for the purpose of gathering ideas and insights for my own volleyball classes. I will not be coaching, supervising, instructing, or otherwise responsible for any athletes present at the Clinic, and I have no relationship to or responsibility for any athlete attending.

I understand that being present at an athletic facility or event involves inherent risks, including but not limited to physical injury, accidents, or illness, even when observing rather than participating in drills. I voluntarily accept full responsibility for any risks, injuries, damages, or losses that may occur as a result of my presence at or attendance of the Clinic.

To the fullest extent allowed by law, I hereby release and hold harmless Coach Chijo LLC, and their employees, contractors, coaches, volunteers, agents, and affiliates from any and all claims or liabilities, whether caused by negligence or otherwise, arising from my attendance at or observation of the Clinic.

2. 📸 Media Release and Use of Likeness

I understand that Coach Chijo LLC may photograph, film, or otherwise record Clinic activities, and that I may incidentally appear in such media as an observer. I grant Coach Chijo LLC, and their legal representatives, licensees, assigns, and affiliates, the unrestricted, royalty-free, and perpetual right to use any media they individually capture of my image, voice, and likeness in any format or media, including but not limited to:

• Social media (e.g., Instagram, TikTok, Facebook, YouTube)
• Promotional or educational materials
• Websites, advertising, and newsletters
• Online courses, webinars, or digital products

Coach Chijo LLC retains full ownership and usage rights to the content they produce. This media may be used for commercial, marketing, instructional, or other lawful purposes. I understand that no compensation will be provided for the use of such content.

3. ✅ Consent and Acknowledgment

By signing this document, I confirm that:

• I am an adult coach requesting to audit the Clinic in my own independent capacity.
• I am not attending as, and have no responsibility for, any athlete participating in the Clinic.
• I have read and understand the Waiver and Media Release above.
• This agreement is binding on me and my heirs or legal representatives.
• I have full legal authority to provide this consent on my own behalf.
