📄 Coach’s Clinic Waiver, Release of Liability, and Media Consent
Please read carefully before signing. This is a legal document.
1. 🛡 Participation Waiver and Assumption of Risk
I, the undersigned adult participant (“Coach”), hereby voluntarily agree to participate in the volleyball coach’s clinic
hosted by Coach Chijo LLC (the “Clinic”), held at Maze Model LLC dba The Bold Break.
I acknowledge that although I may not be actively competing, my participation as an observer, demonstrator, or sideline
participant involves inherent risks. These risks include, but are not limited to, being struck by volleyballs, slips and falls,
accidental collisions, or other physical incidents.
I freely and voluntarily assume all such risks, both known and unknown, and accept full responsibility for any resulting injuries,
damages, or losses that may occur in connection with my participation.
To the maximum extent permitted by law, I hereby release, waive, discharge, and hold harmless Coach Chijo LLC,
Maze Model LLC dba The Bold Break, and their respective officers, employees, contractors, volunteers, agents, and affiliates
from any and all claims, demands, liabilities, costs, and causes of action arising from or related to my participation in the Clinic,
whether caused by negligence or otherwise.
2. 📸 Media Release and Use of Likeness
I understand that portions of the Clinic may be recorded, photographed, or livestreamed for educational or promotional use.
I grant Coach Chijo LLC, and its representatives, licensees, and assigns, full permission to use my image,
voice, likeness, and participation in any form of media, including:
- Social media (e.g., Instagram, TikTok, Facebook, YouTube)
- Training materials and digital courses
- Advertisements, websites, and email newsletters
- Webinars, replays, or future educational content
I acknowledge that these materials may be used for commercial, instructional, or promotional purposes and that I will receive no
compensation for such use. All rights to the media will remain with Coach Chijo LLC.
3. ✅ Acknowledgment of Understanding and Consent
By signing this waiver, I acknowledge the following:
- I have read and understood the terms of this Waiver and Media Release.
- I understand the legal consequences of signing this document.
- I am at least 18 years of age and legally competent to sign this agreement.
- I voluntarily agree to the terms stated above and intend for this agreement to be legally binding upon me, my heirs, and any legal representatives.
